Who is the service for?
We mainly support older people, but all referrals are assessed on an individual basis.
What services does the unit offer?
We offer:
- assessment and rehabilitation/reablement as required to support someone (may include the use of assistive technology and could result in signposting or referrals to a wide range of supports on discharge)
- support for an individual who may be in crisis in their home and who requires a period of assessment (this can prevent an unnecessary hospital admission)
- support following a stay in hospital to continue a period of assessment of need or provide rehabilitation (so that hospital stays are not extended beyond the medical reason for admission)
- facilitating a period of planned respite to support individuals or their carers (which has been assessed for by the locality team)
How is the service accessed?
Access to the service is through social work from either the social work locality or hospital teams.
