Use this form to request a new memorial bench or plaque.

Before you start

Areas with a high number of benches will not be considered.

If an area has benches, no more than half can be memorial benches. This is to stop the area becoming a dedicated memorial garden.

Locations could be areas which lack seating or are a likely stopping point or rest point.
