The plan sets out the partnership’s key priorities and residents are urged to share their views on how health and social care services should look over the next three years.

It’s important that we hear from a wide range of patients, people who use services, and carers so they can play a part in the development of the plan.

Six locality profiles are available to give background and information around the population, health and needs of the people living in each of the localities.
