There is a problem with some of our online forms

We are aware of an issue that is preventing some online forms with maps from being submitted.

We are working to fix this as soon as possible.

Disabled card

How to apply for a disabled card

You can be eligible for a disabled National Entitlement Card from the age of five years and over.

If you are applying for a disabled card for the first time, or if your card has expired and you are re-applying, you can now do this online:

The card and application process are managed by Strathclyde Partnership for Transport (SPT).

What to do if you can't apply online

If you can't apply online, application forms are available from an SPT Travel Centre.

There are no Travel Centres in North Lanarkshire although the mobile travel centre occasionally visits the area. Details are available on the SPT website.

What you'll need to apply

You will need:

  • to provide a recent passport-style colour photograph
  • current documentary evidence of your age and disability
  • proof of your current address

Full details of acceptable types of documentary evidence can be found on the front page of the application form.

If you're not sure about any aspect of the application process, please contact the SPT Travel Card Unit on 0141 333 3211 or visit the SPT website.
