Adult carer support plans and young carer statements
Carers can request an Adult Carer Support Plan (ACSP) at any point, and when someone being cared for is being assessed or reviewed by Social Work, an ACSP must be offered. The ACSP helps identify caring related needs and how these can best be met.
Where caring-related need is likely to be low or moderate, ACSPs can be progressed by Lanarkshire Carers, and where caring related need is likely to be substantial or critical, Social Work Adult Services would progress the ACSP. Where caring related need is complex and can't generally be met in other ways, a carer budget can be agreed.
